grips

hi, always come here when i need an answer and always get one. here i am again. the little black piece with the screw in it at the beginning of the throttle grip will not stay tight. it is forever coming loose. does anyone have a solution. also can the grips be changed and if so , is it hard to do. many thanks and happy riding to all.
